Context is most vulnerable when nobody remembers that the record once needed it.
Negative records gain authority through repetition
Once an error enters a system, later actions can cite it. Additional scrutiny may produce more notes, reviews, and flags. These new records make the original concern appear independently confirmed.

Correction requires locating and updating every derivative record. The person may not even know where the information traveled.

Context may be treated as an excuse
Institutions value consistency. Allowing context to modify every recorded error can appear to weaken standards or invite manipulation.
An employee may hear explanations such as illness, caregiving, system failure, confusing instructions, or emergency conditions and wonder whether accepting them would be unfair to people who complied.

A more reliable memory system can include:

- automatic review or expiration dates;
- distinctions between allegation, finding, and resolved issue;
- propagation of corrections to connected systems;
- human review before old records determine major outcomes.
The goal is not to turn every database into a biography. It is to prevent a compact negative label from claiming more certainty than the underlying event supports.
